Information We Collect Directly From You. The personal information we collect directly from you depends on how you interact with us and our Services.

This may include, but is not limited to, the following: name, photo, username and password, email address, date of birth, gender, payment information, general location data, information or documentation related to your health (e.g., your height, weight, blood pressure, blood glucose), medical test results, and similar information.
    • Wearable or other connected devices: We also may collect personal information when you use a device that is connected to the Internet, such as a heart rate monitor, activity tracker, and/or other devices or wearables that integrate with the Services through our App. When you use a wearable or connected device or product, we may also collect certain information about the device or product, such as its serial number, Bluetooth address, or other unique device identifier.

Protected Health Information. If you authorize your healthcare provider or other covered (entity subject to the Health Insurance Portability and Accountability Act (“HIPAA”)) to share your protected health information (“PHI”) with us, we may use or disclose that information consistent with the purpose(s) for which it was provided. Information We Collect Automatically. We may automatically collect information about your use of our Services, through cookies, pixels, web beacons, and other technologies, including:

  • Your device functionality (including browser type and operating system, hardware, mobile network information), IP address, and other unique online or device identifiers;

  • To enable service providers or vendors to assist us in operating our business, or to share information with our affiliates consistent with the purpose(s) for which the information was provided;

  • To protect our interests, rights, safety, or property or that of others;

3. Your Choices

You have the right at any time to opt out of the receipt of promotional email and other commercial communications. You may do so by following the unsubscribe instructions in each such email, or by contacting us at livebetter@bellsant.com.

A note regarding mobile push notifications: We may ask for your consent to contact you via push notification on the App. You can manage the types of push notifications you receive from us by modifying the settings within the App. You may also set your mobile device settings to not allow push notifications from us.

4. Cookies and Other Tracking Mechanisms

We and our service providers [or other third parties working on our behalf] may use cookies and other tracking mechanisms to track information about your use of this website and the App. Such information may include your device’s IP address, browser type, operating system, computer platform, mobile device type, mobile service provider, unique device identifier, web pages viewed, and/or the date and time of your visit.

Our Services are not intended for children under 16 years of age, and we do not knowingly collect personal information from children under 16. If you are under 16, do not use or provide any information through this website, the App, or on or through any of their features. If we learn we have collected or received personal information from a child under 16 without verification of parental consent, we will promptly delete that information.

8. Transfer of Personal Information

We are based in the United States, and our Services are designed for users in, and operated by us from, the United States. If you reside outside the United States and use our website, App or otherwise provide information to us, you agree that your information will be transferred to and processed in the United States for the purposes described in this Privacy Policy, which may provide different levels of privacy protection than the jurisdiction in which you are located.

9. HIPAA

To the extent we may receive PHI directly from a covered entity subject to HIPAA, BellSant is a business associate (as defined under HIPAA). We will receive PHI from a covered entity only with your prior written authorization. We will execute all necessary and appropriate agreements with the relevant covered entity or entities, as required by law, and will implement and maintain appropriate safeguards, consistent with the HIPAA Security Rule to prevent the unauthorized use or disclosure of PHI (including electronic PHI).

The products and/or services provided by BellSant or through the Services are to be used for informational purposes only. No information, Technology, or Service provided by BellSant constitutes medical advice or the practice of medicine, and is not to be relied upon by visitors to this website or the App, users of our products or services, or any other individual for diagnostic purposes or for the treatment of human illness. The information and services provided on this site, the App, and through our products and services is not intended to be patient education, does not create any patient-physician relationship, and should not be used as a substitute for professional diagnosis and treatment. If you are an end-user of the Service, you should consult a healthcare provider before making any healthcare decisions or for guidance on any specific condition, and the diagnosis or treatment thereof.
